Six dimensions. One score.
Over four weeks, Duaxiom's two founders examine how your organization really works across six key areas. We uncover what is slowing decisions, why the same problems keep returning, and where too much depends on too few people.
You get one Operational Health Score, the evidence behind it, and a clear, ranked plan showing what to fix first, what can wait, and where change will make the greatest difference.
See how it works- Decisions mostly move at pace
- Ownership is mostly clear
- Reporting exists, but isn't fully consistent
- Technology helps more than it hurts
What we actually measure.
Strategic Clarity
We assess how clearly the organization's mission translates into daily decisions, and whether leadership holds a shared view of priorities and direction.
Governance & Decision Rights
We examine how decisions are made and by whom, how policy is set and reviewed, and how leadership is held accountable.
People & Org Health
We evaluate culture and psychological safety, performance management, and key-person dependency across the organization.
Financial Stewardship
We review the accuracy and timeliness of financial reporting, revenue concentration, and the strength of budgeting and reserve practices.
Operating Rhythm
We look at how priorities are set and tracked, how effectively meetings translate into decisions, and how reliably information moves through the organization.
Technology & Tools
We assess system integration and data reliability, current use of AI and automation, and the organization's security and continuity posture.
Most organizations aren't broken. They're tangled.
- Simple decisions take three meetings.
- The same five arguments, on repeat.
- A tech stack that costs a fortune and does nothing.
- A leader who knows it's broken — but not where.
- Unchecked, it doesn't kill you — it traps you.
- Good teams work hard and still can't get ahead.
- Leaders feel overloaded but can't find the source.
- Scaling vs. stalling is rarely strategy — it's operational.
An Operational Health Score across six dimensions.
Every engagement begins with a structured diagnostic. We score what's working, what's not, and what to fix first.
The diagnostic is the product. Multi-session, partner-delivered, scored against a proprietary rubric we've built across schools, owner-led businesses, and scaling companies.
Each dimension is rated and weighted by track. The output is an Operational Health Score, a prioritized action plan, and a board-ready report you can act on the same week.
Most clients use the diagnostic as the entry point to a longer engagement. Some take the report and run with it themselves. Both are fine — we built it that way on purpose.
Every dimension we score sharpens the rubric.
What the report actually surfaces.
A diagnostic scores each dimension, then ranks findings by impact. Often the highest-impact issue isn't the one leadership expected — in the sample above, a revenue-concentration risk sitting behind healthy top-line growth.
From there, the report lays out a prioritized action plan: what to fix first, what it should move, and how to track it — the difference between an organization that keeps scaling and one that quietly stalls.
This is an illustrative example — a worked example of the output, not a specific client engagement.
The Operational Diagnostic starts at $12,500.
A clear floor, bespoke from there. Most engagements begin with a 30-minute call — no prep required.
No commitment — we'll tell you honestly if we're not the right fit.
